After we receive this form, we will contact you within a week to schedule a family interview, if we’ve not already met with you. A $250 deposit is due alongside the application (per family). Please mail a check made out to “Sursum Corda Classical” to 143 Washington St., Oregon, WI 53575.

In filling out this form, if and once enrolled, unless otherwise specified in writing to the headmaster, you grant Sursum Corda the right to photograph or record your student and use pictures and media for promotional purposes.
